The forms included here are for your convenience. Application.pdf is to apply for a shoot date, cancellation.pdf is to cancel a date, and shootreport.pdf is to fill out after the shoot. Use them at your discretion. Different operating systems handle them in different ways. You can use the email submit button to send them and the print button for your records or just the print button and then fax them. If you find them inconvenient or do not wish to use any, or all, of them, that is fine. If this ends up being more trouble than help, I will let you know.
The email submitted form only sends the data you enter and not the complete form. Some operating systems will require you to save your submission to another place and attach it to an email. Some systems send it directly. It must come to me with an .xml file extension. In my experimenting with them, some had trouble but most did not. The best file to save them to, if required, is a document file. If I receive an email from you I cannot use, I will contact you.
